Dental Plan

We are pleased to offer you and your family high-quality dental coverage to help you maintain your best health.

Our dental plan is provided through the Anthem National Dental Network, which is often listed as Anthem Blue Cross Blue
Shield. This plan gives you access to a large network of providers. If you need to find an in-network dentist, simply visit
anthem.com.

You have two choices for coverage: an Employee Plan or a Family Plan.

While Anthem provides the network, the day-to-day work of processing claims and administering the plan is handled by our
third-party administrator, Point C.

Important: If you have any questions about a claim, need assistance with your coverage, or re4ceive a dental bill, your fi8rst
step should always be to call Point C directly. They are your primary resource for all administrative issues.

Any dental cards or official bills you receive will be mailed to you directly from Point C.

The plan covers many necessary dental services, though the amount the plan pays depends on the service - you will have
different member cost shares (deductibles, co-pays, etc.). For a complete, detailed breakdown of covered services and your
specific costs, please consult the Employee Benefits Overview (EBO) document, available on page 9.

Finally, if you are enrolling dependents, coverage is available for them through the end of the calendar year in which they turn
age 26.
