Curriculum Plan For Declaration of Major

This form is to be completed when declaring or changing a major. If changing a major please fill out this form as well as the Change of Advisor/Major/Minor/Concentration Form.

You are expected to work with your advisor(s) to complete a term-by-term curriculum plan in the space below. The plan must include all courses required to complete the General Education requirements, major(s) and minor if a minor is sought. If this plan will require a total of more than eight regular (fall or spring) terms, you must submit an Extension of Terms Request form and accompanying letter with this document. The Extension of Terms Request form may be obtained in the Student Self Service room, 101 Lincoln Hall.

Any subsequent change to this curriculum plan (adding a second major or a minor, e.g.) necessitating additional terms will require the completion and submission of a new curriculum plan and an Extension of Terms Request form.

Begin with your next term of enrollment and include all terms (including Summer terms) in which you plan to take courses. Provide the course prefix, number and title for all courses, e.g., GSTR 332, Sci. Knowl. & Inquiry. Elective courses may be indicated by “Elective” or by the name of the course.
