    • Apple Device Request Form – Used for employees and supervisors to request the purchasing of Apple computers and Tablets. (This form is not for requesting Apple iPhones or iPad that need cellular data. Please use the Mobile Device form for those requests.)
    • Mobile Device Request Form – Used to request and obtain approval for college employees who are eligible for a College cellphone or iPad with cellular data.
    • Server Request Form – Used to request and obtain approval to have a server necessary for the day-to-day business of the College deployed into the IS&S environment.
  • Service Account Forms
      • Non-Employee Account Request Form – Used to request email and Active Directory accounts for contractors, temporary employees, and third-parties requiring a valid account. (Sponsor completes the form, Non-Employee needs to visit HR with Valid ID)
    • Useful Campus Technology Information – This document provides a list of commonly used and helpful websites affiliated with Berea College.  Many of the listed sites are required for day-to-day work/study on campus and many will help increase efficiency.
    • Technology Purchasing Guide – Recommendations for technology equipment that can be purchased by individual departments.  These items include but are not limited to: external hard drives, computer monitors, and LCD televisions.
    • Campus Voicemail Pin Reset Self-Service – This self-service is available for Berea College faculty and staff to reset on-campus voicemail. This resource is only available on campus or by VPN. Please contact the IS&S Help Desk if additional assistance is needed.
