How to Use the Forms
To use the following documents,
you must:
- Click on the download link.
- When asked to open or save the file, click the Open button
and another window should appear.
- In that window, double-click the Word document to open it.
- Before entering any information,
you must select File-->Save
As in the Word document and a Save As window will appear.
Be sure to save the document to your Desktop for easier access
later.
At this point, you have saved the document on your computer;
you can work on it at your convenience.
- You may
either attach the completed document to an e-mail message or
print the document and send it through the regular mail.

Feedback Circle Forms
To complete a Feedback Circle, you will need the following information:
Cover Sheet: This is sent to
the people who will provide feedback for a feedback recipient
- the Feedback Circle Members.
It explains the process and includes a due date. NOTE: When
the feedback form is completed and returned to the Consolidator,
the Cover is no longer needed and is removed from the feedback.
Feedback
Form: This is the form upon which Circle members
put their feedback statements. Please remember the Feedback
Circle
process is intended to be a confidential process. We recommend
that you delete the feedback form after you are sure it
has been received by the consolidator.
Consolidation Form / Development Plan: These
are the two pages upon which feedback is consolidated into themes
and a plan
is created for development. The final Development Plan
is signed
after it is discussed by both supervisor and the feedback
recipient.
Blank Position Description: In
order to provide Feedback Circle Members a reference for
providing performance feedback, a current position description
must be sent to the feedback circle members. You may send a current
copy of the position description from your files. Or, in case
you do not have a copy in your files, you can obtain a copy from
People Services.
Alternatively, you can use the document below to create, revise,
and send the position description electronically. If you do
change the position description, it will need to be signed
appropriately and sent to People Services.
All Feedback Circle Forms - with lines: Use
this if you want all the forms printed for handwritten
responses.
