Requests for Banner Access
 

Access to the Banner Administrative system may not be granted until the following requirements have been fulfilled:

  1. Read the Security and Access Policy.
  2. Complete the Banner Access Application and acquire all necessary signatures.
  3. Send completed and printed access form to Cary Hazelwood at CPO 2208.

Please fill out form completely. Incomplete forms will be returned to the requestor.

  1. Full Name
  2. Campus Extension
  3. Title
  4. Department
  5. CPO
  6. Choose type of access: Staff/Faculty or Student
  7. Choose an Instance:
    1. Production: Current data. Everyone should request access to Production.
    2. Preproduction: Used for testing and training only.
    3. Both: Will give user access to both Production and Preproduction.
  8. Action:
    1. Create New Account: Request creation of a new account.
    2. Modify Existing Account: Add or delete access for an existing account.
    3. Reassign Student Account: Reassign a student login to another student.
    4. Deactivate Account: Temporarily deactivate account until reassigned.
    5. Delete Account: Delete account from system.
  9. User ID. Enter Banner User ID when modifying, reassigning, deactivating or deleting an account.

Access

Banner access is granted by CLASS or FORM. A Banner class is a collection of forms with specific access that are grouped together under one name. A list of Banner classes is available. Enter the name of the requested Banner class(es) in the Class section of the form.

If a user needs access to a particular form, please enter the seven letter form code and the type of access needed (maintenance or query) under the Individual Form Access section.

Alternately, access can be copied from one user to another. If a user needs the same access as another user, please enter the name of the user to be copied in the “Same access as:” field. This type of access requires signatures for each object that the other user has as well.

Print

Once you have completed the form, print the form.

Required Signatures

  1. Your signature.
  2. Your supervisor’s signature.
  3. Signature(s) of each functional liaison from which you are requesting access (e.g., if you are requesting Student and Finance access, you would need signatures from both liaisons).
