Students
wishing to notify the College of an address or name change may
do so by submitting this Change of Address / Name form to the
Student Service Center (First Floor Lincoln Hall or CPO Box 2168).
This
change will be entered into the college’s student database
and the new information will be available to all who have access
to that data.
